Extensive basal cell carcinoma and disseminated lesions in Hand-Schüller-Christian disease.

چکیده
Sir, Langerhans’ cell histiocytosis (LCH) includes four clinical entities with a common characteristic of infiltration of LC in different tissues. Biopsy of suspicious lesions and staining for CD1a and S100 protein or antilangerin are needed to establish a definitive diagnosis of LCH. Electron microscopy to identify Birbeck granules is performed less frequently because of time and cost constraints (1, 2). The pathogenesis remains unclear. A neoplastic aetiology has been suggested due to the existence of a single clone in the infiltration, although no cytogenetic abnormalities have been found (3, 4). Most investigators consider LCH to result from an immunological dysfunction (2, 5, 6). The presence of aggregates of other immunologically active cells, decreased numbers of suppressor T lymphocytes and high levels of cytokines in the lesions suggest an exaggerated physiological response of LC to an antigen, or an appropiate response to abnormal signals from other immune cells (1, 7, 8). Although numerous facts are known about the cell mediators, it has not been possible to prove the participation of any factor in the unchaining of the clonal proliferation of the cells in the LCH (9). We present here a new case of basal cell carcinoma (BCC) in association with one type of LCH, HandSchüller-Christian disease, with a probably late specific cutaneous infiltration of the LC.

Langerhans cell histiocytosis (LCH) is a rare disease, of unknown pathogenesis, characterized by intense and abnormal proliferation of bone marrow-derived histiocytes (Langerhans cells). It can present both local and systemic manifestations involving bone, skin and mucosal tissue, and internal organs. Three basic clinical forms develop: Letterer-Siwe disease (subacute or acute disseminated form...
متن کاملIncidence of incomplete excision in surgically treated basal cell carcinomas and identification of the related risk factors
Background: Surgery is the most frequent treatment modality for basal cell carcinoma but in spite of its high cure rate, the frequency of incomplete excision varies widely (0.7-50%) among dermatologic centers. Our case series was designed to determine the frequency of incompletely excised basal cell carcinoma and the related risk factors. Methods: A total of 1424 basal cell carcinoma (1040 pati...
متن کاملLangerhans cell Histiocytosis: Report of a case with cutaneous lesions with favorable response to oral Thalidomide
We report of a 29-year-old man presented with yellow-red, scaling papules on the skin of scalp, retro-auricular folds, inguinal, axillary, chest and abdomen. The lesions had a poikilodermatious appearance in some areas, such as trunk. The patient also had diabetes insipidus and skull osteolytic lesions. The diagnosis of langerhans cell histiocytosis (LCH) was suggested based on H and E staining...
متن کاملTreatment of Advanced Carcinoma of the Larynx and Hypopharynx with Laser Followed by External Radiotherapy
Introduction: Radical laryngeal surgeries for extensive laryngeal and hypopharyngeal tumors often require a permanent tracheostomy, which has an immense impact on the quality of life of patients. A minimally invasive technique such as transoral laser microresection (TLM) followed by radiotherapy can preserve the functions of the voice and swallowing. The aim of this study is to evaluate the ro...
متن کاملAtypical (Fungiform) basal cell carcinoma arising on a Tattoo: A case report
Basal cell carcinoma (BCC) is a malignant tumor that may arise in skin damaged by sunlight and ionizing radiation in burn or vaccination scars. Clinical variants of BCC include nodular, morphea type, superficial and polypoid. Tattoo reactions are histologically diverse and can be classified in three main classes: 1) Allergic, granulomatous, and Lichenoid reactions; 2) Inoculation and infe...
